participates in all commercial insurance plans. We accept all
medicare and medicaid HMOs. We accept Tricare standard,
but do not accept Tricare Prime. We also offer private
fee for service.
As a courtesy to our patients we will file insure forms. We
require referrals from your primary care physician or specialist
for pain care. It is the patient's responsibility to acquire
these referrals. Please be aware that you are responsible for
your bill if you do not obtain the proper referral. All co-pays
for HMO and PPO type insurance plans are due at the time of your
visit.
Most physicians will provide you with referrals to our
practice. If you have difficulties in obtaining referrals,
please go to the nearest
Doctors Care or
Doctors Express urgent care facility to obtain the referral
order. Please
download this form and
give it to your referring doctor. We will be glad to assist you
in obtaining necessary referrals from your doctors or from one
of our affiliates. Please call us for assistance.
Medicare/Medicaid: We participate in Medicare and Medicaid.
Medicare patients will be required to pay the 20% co-pay at the
time of their visit. Our office will file your claim promptly
