To help make your first visit with us effective and
productive, there are several items that you should bring with you. This helps
us make sure that your first visit with us is thorough and complete.
Please plan on bringing the following items to your visit:

MRI, CT, Myelogram
Films. Obtain these films from the facility it was
done prior to the visit. We wish to see the actual films.

Medical records from prior visits that you think would be helpful.

A List of current medications. A
List of current medications and medication bottles ( we need to inspect your
medication bottles, and count the left over pills before we prescribe
medications)

Insurance information and cards
Please do not assume that we have
received above mentioned documents from your referring doctor. You may
call our office prior to your clinic visit to check on the documents we have
gathered from your referring doctor. We may request for additional
appointments if we don't have all documentation needed for decision making at
the time of initial evaluation.
We realize your time is valuable, and we will make every
effort to remain on schedule. Occasionally, our physicians may be disrupted by
medical emergencies. When this occurs, we hope you will understand and remain
patient. Of course, we will make every effort to inform you of unusual delays.
Arriving on time for your appointment will help with any
unnecessary delays. If you find that you will be unavoidably late, please
contact the office. Should you be unable to keep your appointment, please notify
our staff at least twenty-four hours in advance.

fact we don't print patient questionnaire any more, we are a paperless office!

What should you expect?
As a patient you will receive a full evaluation from Dr.
Thiyaga, a specialized spine physician, who will identify the
diagnosis and treatment options. Patients could be seen within 7 working days of
referral by your doctor.
Every patient receives a treatment plan created
specifically for him/her which is directly delivered by Dr. Thiyaga. We do not
have interns or trainee physicians learning in our private clinic, so you can be
assured of the highest quality care at all times by a Board Certified and
Fellowship tr ained physician. This treatment goes beyond the basic pain
management by employing therapeutic methods that treat the specific source of
the problem, providing a specific treatment program for reduction in pain and
improvements for a more active lifestyle.
